Parent consent for enrollment

Complete your child’s registration, type your full name and check the parent-consent box. Submit a separate registration for each child; you can use the same guardian email.

Notice to parents

Taqwa Institute, 1642 S Parker Rd, Suite 205, uses the student’s age, name, grade, gender, learning format, submitted work and grades to provide lessons, place students and return teacher feedback. Guardian contact details support enrollment and safeguarding. Student records are private to the student and authorized staff. Course files and grades are also stored with Google in the institute account tquranarabic@gmail.com; the portal is hosted through Sites and Cloudflare.

Multiple-choice work is scored against fixed answer keys. Teachers assess other work and can review a score. Work from students under 18 is not sent to OpenAI. We do not sell student information or use religious-learning records for advertising.

We record your typed name, the consent date, the applicable privacy-notice version and the child’s account reference. A typed name records your declaration; it does not independently verify your identity.

You can review, correct or withdraw consent, or request deletion by contacting tquranarabic@gmail.com or 720-466-0722. Staff verifies the request and coordinates removal from the site and Google copies. Read the privacy and retention notice.
